Personal data notice (GDPR)
The data controller is FHU Relax Dorota Banaszek, which runs the Świat Masażu i Relaksu salon (ul. Kacza 9 lok. H, 01-013 Warszawa; swiatmasazuirelaksu@wp.pl; tel. 884 087 999). We process the data from this form (name, phone, service, date, notes) to answer your request, confirm your visit and keep your loyalty card (Art. 6(1)(b) GDPR). Providing data is voluntary, but we cannot handle your request without it. You do not need to include health information in the notes – if you do, we will use it only to perform the treatment safely (Art. 9(2)(a) GDPR). We keep the data until your request is handled and, for the loyalty card, for up to 24 months after your last visit. Only the salon and our hosting and e-mail providers have access to it. You have the right to access, rectify, erase, restrict and port your data, to withdraw consent, and to lodge a complaint with the Polish supervisory authority (Prezes UODO). Privacy policy
